Dr. Giriraj Gandhi

Midface Lift (Hammock Lift)

A highly strategic elevation of the cheek and midface structures to restore youthful volume, lift sagging skin, and enhance deep facial dimension.

What Is A Midface Lift?

The midface is the architectural center of youth. As we age, the malar fat pad (the volume of the cheek) begins to detach and slide downward. This descent creates a cascade of aesthetic issues: it hollows the under-eye area, flattens the cheekbones, and creates heavy, deep nasolabial folds (smile lines) around the mouth.

A Midface Lift, often utilizing an advanced 'Hammock Lift' technique, addresses this specific anatomical drop. By making tiny incisions hidden within the hairline or inside the mouth, Dr. Gandhi physically captures the descended cheek fat and suspends it vertically back to its youthful position across the cheekbone, instantly restoring the 'apple' of the cheek and smoothing the transition from the eye to the lower face.

What Concerns Does A Midface Lift Address?

What Concerns Does A Midface Lift Address?

  • 01Deep, heavy nasolabial folds (the lines from the nose to the mouth)
  • 02Flat or deflated cheekbones lacking youthful prominence
  • 03Severe hollowing or 'tear troughs' directly beneath the eyes
  • 04A tired or drawn appearance isolated to the center of the face
  • 05Early signs of jowling caused by midfacial tissue descent

Cheek Fillers vs. Surgical Midface Lift

In your 30s, hyaluronic acid fillers (like Voluma) placed on the cheekbones can beautifully camouflage early volume loss and create the illusion of a lift.

However, by the time you reach your late 40s or 50s, the physical weight of the tissue has dropped significantly. Pumping more filler into a fallen cheek will only result in 'pillow face' and cause the tissue to sag further under the added weight. A Midface Lift is the only structural solution to physically elevate the anatomy and restore true, three-dimensional youth without looking 'injected.'

The Dr. Gandhi Difference

The Dr. Gandhi Difference

A standard lower facelift primarily targets the jawline and neck, often doing very little for the center of the face. Conversely, relying solely on cheek implants can look rigid and unnatural.

Dr. Gandhi treats the midface dynamically. Using an Endoscopic or deep-plane suspension approach, he creates an internal biological 'hammock.' He secures the fallen malar fat pad to the strong, immovable fascia near the temples using dissolving micro-sutures. This strictly vertical lift avoids the 'pulled' or 'windblown' look, ensuring your cheeks move naturally when you smile while maintaining a flawless, elevated resting contour.

Dr. Gandhi's Insider Tip

"If you try to fix heavy smile lines by just injecting them with filler, you end up with a heavy, puffy lower face. The problem is not a lack of volume; the problem is that the volume has fallen. You must lift the cheek back up to pull the smile line smooth."

The Endoscopic Midface Lift

Tiny incisions are hidden within the temporal hairline. Utilizing a micro-camera, Dr. Gandhi gently releases the midface tissues and lifts the cheek pad vertically, anchoring it securely to the deep temporal fascia. Excellent for younger patients needing a pure vertical vector.

Trans-Blepharoplasty Approach

If a patient is already undergoing lower eyelid surgery (Blepharoplasty), Dr. Gandhi can seamlessly access the midface through the same invisible incision just below the lash line, pulling the cheek pad upward to perfectly blend the eye into the cheek.

Integration with Lower Facelift

For comprehensive rejuvenation, the midface lift is often performed concurrently with a Deep Plane Facelift, allowing the surgeon to harmonize the entire face from the cheekbones down to the collarbone.

Will a midface lift change the shape of my eyes?

A properly executed midface lift supports the lower eyelid, actually preventing the 'pulled down' look that aging causes. It restores the natural, youthful almond shape of the eye by providing a strong shelf of cheek volume immediately beneath it.

How long does the lift last?

Because it is a deep structural repositioning of your native tissue, the results of a midface lift typically last 10 to 15 years. You will continue to age naturally, but you will always maintain a more elevated contour than if you had not had the surgery.

Are the incisions visible?

No. The incisions are masterfully concealed either completely behind the hairline at the temples, or completely inside the mouth (intraoral approach), depending on the specific technique Dr. Gandhi determines is best for your anatomy.

Phase 1: Filler Dissolution

If you have had extensive dermal fillers placed in your cheeks or tear troughs, Dr. Gandhi will require you to dissolve them with Hylenex a few weeks prior to surgery so he can accurately assess and lift your true biological anatomy.

Phase 2: Medical Clearance

Cease all blood-thinning supplements, including aspirin, NSAIDs, fish oils, and vitamin E, at least two weeks before surgery to minimize the risk of facial bruising and hematoma.

Phase 3: At-Home Care Setup

Prepare a recovery station where you can sleep comfortably on your back with your head elevated at a 45-degree angle. This is critical for the first week to allow gravity to pull swelling away from the midface.

The First 72 Hours

Your cheeks will look significantly over-corrected and sit very high on your face. This is intentional and necessary. Swelling and mild bruising around the eyes and cheeks are highly common. You must adhere to a soft diet and avoid excessive smiling or extreme facial expressions.

Days 4 to 10

The intense swelling begins to resolve. You may feel a sensation of tightness in the cheeks and temples, which indicates the internal suspension sutures are holding securely. Most patients feel comfortable returning to remote work or light desk duties by the end of the first week.

Weeks 3 to 6

The cheeks will drop slightly and settle into their final, natural, and beautifully contoured position. You can confidently return to all social events and resume wearing standard makeup and contact lenses.
