Dr. Giriraj Gandhi

Upper Blepharoplasty

A microscopic architectural refinement of the upper eyelid to eliminate heavy, hooded skin, restoring a crisp, alert, and effortlessly youthful gaze.

What Is Upper Eyelid Surgery?

The skin of the upper eyelid is the thinnest on the human body, making it exceptionally vulnerable to gravity and aging. Over time, this delicate tissue loses elasticity and begins to drape downward, obscuring the natural eyelid crease, resting heavily on the eyelashes, and casting a perpetual shadow over the eyes that conveys exhaustion or sadness.

Upper Blepharoplasty is an elegant, millimeter-precise surgical intervention. Dr. Gandhi meticulously excises this exact redundancy of skin and sculpts any bulging medial fat pads. The incision is then flawlessly closed deep within your natural eyelid crease. The result is not an altered eye shape, but the profound re-awakening of your natural eyes, allowing light to hit the iris and restoring a vibrant, rested expression.

What Concerns Does Upper Blepharoplasty Address?

What Concerns Does Upper Blepharoplasty Address?

  • 01Severe 'hooding' where excess skin completely hides the upper eyelid platform
  • 02Skin that physically rests on the eyelashes, causing a heavy, tired sensation
  • 03Difficulty applying eye makeup or eyeshadow due to a loss of lid space
  • 04Obstruction of the superior (upper) visual field in severe cases
  • 05A constant appearance of fatigue, anger, or sadness despite feeling energetic
  • 06Bulging fat pads in the inner corners of the eyes near the nose

Botox Brow Lift vs. Upper Blepharoplasty

If your eyelid heaviness is extremely mild and caused by a descending eyebrow, a few units of neurotoxin (Botox) placed strategically can create a temporary 'chemical lift,' raising the brow and clearing the eyelid space for 3 to 4 months.

However, if the physical skin of the eyelid has permanently stretched and lost its elasticity, no injection can shrink it. In these cases, true structural relief requires the surgical excision of the redundant skin. Oftentimes, Dr. Gandhi will evaluate whether you need an Upper Blepharoplasty, a Surgical Brow Lift, or a combination of both to achieve the perfect result.

The Dr. Gandhi Difference

The Dr. Gandhi Difference

The most common error in upper eyelid surgery is aggressive over-resection-removing too much skin or fat. This leaves the patient looking 'startled,' skeletal, or unable to close their eyes completely, irreversibly damaging the ocular surface.

Dr. Gandhi champions a highly conservative, tissue-sparing philosophy. He meticulously preserves the orbicularis oculi muscle to ensure powerful, complete blinking. His incision design is an art form, mathematically calculated to fall exactly into your eye's natural anatomical fold. By prioritizing volume preservation and micro-precision suturing, he guarantees a result that is brilliantly refreshed, functionally perfect, and completely un-operated in appearance.

Dr. Gandhi's Insider Tip

"A beautiful upper eyelid isn't completely hollow. Youth is defined by fullness. Our goal is to simply 'crisp up' the lid platform so makeup sits perfectly, while leaving enough natural tissue to ensure your eyes retain their soft, elegant character."

Precision Skin Marking

Before any anesthesia is administered, while you are sitting upright, Dr. Gandhi uses specialized calipers to mathematically mark the exact millimeters of excess skin, ensuring absolute symmetry and protecting your ability to close your eyes.

Tissue Excision

Under local anesthesia, the redundant skin is delicately removed. If there is a bulging fat pad in the inner corner (the medial fat pad), it is gently contoured to eliminate puffiness.

Invisible Closure

The incision is closed using hyper-fine, hair-like sutures placed directly inside the natural fold of the eye. Once healed, the scar is completely hidden when your eyes are open, and virtually imperceptible when closed.

Is the procedure performed awake?

Yes, Upper Blepharoplasty is highly suited for our clinical suite. It is comfortably and painlessly performed under local anesthesia (similar to a dental procedure) with a light oral sedative to keep you completely relaxed. The surgery typically takes less than an hour.

Will insurance cover this?

If the excess skin is so severe that it hangs over your eyelashes and obstructs your superior vision, it may be deemed a medical necessity. We will perform a formal visual field test during your consultation to determine if insurance authorization is possible.

When can I wear eye makeup again?

You must keep the incisions completely clean and free of cosmetics while the sutures are in place. Once the sutures are removed at day 7, you can typically resume wearing eyeshadow and mascara by day 10 to 14.

Phase 1: Medication Review

The eyelids are highly vascular. You must cease all aspirin, ibuprofen, fish oils, vitamin E, and green tea extracts exactly two weeks prior to surgery to minimize surgical bleeding and prevent severe post-operative bruising.

Phase 2: Lash Extensions

If you wear false eyelash extensions, they must be completely removed prior to surgery to ensure the surgical field is pristine and to prevent the surgical drapes or ointments from pulling on them.

Phase 3: The Ice Station

Purchase gel ice masks or small bags of frozen peas. Aggressive icing during the first 48 hours is the single most important factor in ensuring a rapid, bruise-free recovery.

The First 72 Hours

Your eyelids will feel tight and slightly swollen, but sharp pain is incredibly rare. You must sleep with your head elevated on 2-3 pillows and apply ice continuously (20 minutes on, 20 minutes off). You will apply a prescribed antibiotic ointment to the incisions.

Days 4 to 7

Swelling begins to rapidly subside. Mild bruising may appear on the upper lids or migrate to the lower lids. You will return to the clinic around day 7 for the completely painless removal of your ultra-fine sutures.

Weeks 2 to 3

The incisions will appear as faint pink lines hidden in your crease. You are 'restaurant ready' and can return to normal social activities. The crisp, beautiful new contour of your upper eyelid is permanently revealed.
